Hercule Poirot, en mission au Moyen-Orient, doit regagner Londres en toute urgence. Il prend place à bord de l'Orient-Express, à Istanbul, anormalement bondé en cette période de l'année. Durant le voyage, il est approché par Samuel Ratchett, un riche Américain qui lui demande sa protection. Poirot refuse poliment. Le lendemain, le corps de Ratchett est retrouvé. L'homme a été tué de douze coups de couteau. Poirot s'interroge sur ce meurtre étrange où tous les indices partent dans des directions opposées. Il découvre alors que le vrai nom de Ratchett est Cassetti et qu'il est en fuite à la suite du rapt meurtrier de la jeune Daisy Armstrong aux États-Unis, il y a cinq ans
